Application forms should not be sent directly to the charity. You should complete as much of the form as possible before getting it signed by your GP, physiotherapist (if you are already receiving treatment), or Occupational Health Nurse/FMO.
Forms should then be submitted to your Occupational Health Unit or the Police Federation who will verify you donate to the charity before forwarding the form to The Police Treatment Centres. If your force cannot confirm you donate, you should include two payslips (your most recent and one from 12 months previously) when you send your form to The Police Treatment Centres.
Clinical staff at The Police Treatment Centres assess each application form before allocating a date for treatment via your Occupational Health Unit/Police Federation. Our Eligibility Policy and Clinical Admission Criteria and Clinical Reporting Policy are considered when dealing with all applications for treatment.
Please note that applications forms are only valid for three months from the date of signature.
Please be advised, with effect from 1st March 2016 we will no longer be able to accept any of the old Treatment Application forms.
