How to Apply

Each year about 3500 officers receive treatment on a residential basis at The Police Treatment Centres. By prioritising applications on the basis of clinical need we seek to offer treatment at the optimum time in a patient’s recovery. Treatment is provided free-of-charge to officers who make a payroll giving voluntary donation to the charity.

Application forms for treatment are available from Occupational Health Units and Police Federation representatives. Alternatively you can click here to download the PTC Inpatient Application Form - January 2012 (please note that applications on the old form will no longer be accepted).

Following a number of matters arising during 2011 it has been necessary to introduce a process to assess, as well as support for, the attendance of patients ‘companions’.

In all such cases the applicant must also submit a PTC Companion Application Form providing sufficient information upon which to make a decision to allow a companion to also attend the Centre. This is particularly important where the ‘companion’ themselves may have clinical or special needs that should be taken account of e.g. accommodation, diet, etc.

Application forms should not be sent directly to the charity. You should complete as much of the form as possible before getting it signed by your GP, physiotherapist (if you are already receiving treatment), or Occupational Health Nurse/FMO. It should then be submitted to your Occupational Health Unit or the Police Federation who will verify you donate to the charity before forwarding the form to The Police Treatment Centres. If your force cannot confirm you donate, you should include two payslips (dated six months apart) when you send your form to The Police Treatment Centres.

Clinical staff at The Police Treatment Centres assess each application form before allocating a date for treatment via your Occupational Health Unit/Police Federation. Our Eligibility Policy - Jan 2012 and Clinical Admission Criteria and Clinical Reporting Policy are considered when dealing with all applications for treatment.

An outpatient service may be offered to those officers living or working within an hour's drive time of each Police Treatment Centre (although treatment decisions are based on individual circumstances). Click here to download the PTC Outpatient Application Form - 2012
